Venlafaxine HCl
Indications/Uses
Depression, including depression w/ associated anxiety; anxiety or generalized anxiety disorder, including long-term treatment; social anxiety disorder (social phobia); panic disorder w/ or w/o agoraphobia. Prevention of relapses & recurrence of depression.
Dosage/Direction for Use
Depression 75 mg once daily; may be increased to 225 mg/day in moderate depression & 375 mg/day for severe depression. Generalized anxiety & social anxiety disorder Initially 75 mg/day; may be increased to 225 mg/day at not <4 days interval. Initially 37.5 mg/day for some patients w/ social anxiety disorder. Max: 225 mg/day. Panic disorder w/ or w/o agoraphobia Initially 37.5 mg/day for 4-7 days in outpatients followed by 75 mg/day. Max: 225 mg/day.
Administration
Should be taken with food: Take at approx the same time daily. Swallow whole, do not divide/chew/crush or dissolve in water.
Contraindications
Special Precautions
Increased suicidality. Monitor patients w/ raised IOP, acute narrow-angle glaucoma as mydriasis may occur. Recent MI or unstable heart disease. Routine BP monitoring is advisable. History of convulsion, bipolar disorder, aggression, predisposal to bleeding. Monitor serum cholesterol levels during long-term treatment. Patients taking diuretics or vol-depleted. Do not discontinue abruptly, dose tapering is recommended. Pregnancy & lactation. Childn <18 yr. Renal & hepatic impairment. Elderly.
Adverse Reactions
Asthenia/fatigue, chills; HTN, vasodilatation, palpitations; decreased appetite, constipation, nausea, vomiting; increased serum cholesterol (particularly w/ prolonged administration & possibly w/ high doses), wt loss; headache; abnormal dreams, decreased libido, dizziness, dry mouth, increased muscle tonus, insomnia, nervousness, paresthesia, sedation, tremor, confusion, depersonalisation; yawning; sweating (including night sweats); accommodation abnormality, mydriasis, visual disturbance; abnormal ejaculation/orgasm (males), anorgasmia, erectile dysfunction, impaired urination, menstrual disorders associated w/ menorrhagia, metrorrhagia, increased urinary frequency.
Drug Interactions
Other CNS-active drugs, ethanol, haloperidol, cimetidine, imipramine, ketoconazole, metoprolol, CYP2D6 & 3A4 inhibitors.
